We understand that every patient’s needs are different. That’s why our Patient Support Program (PSP) offers a range of enrolment methods to make the process easy and accessible.

Choose the enrolment method that works best for you:

  • Fax: Complete and print your PPAF/Enrolment form, then fax it to us.
  • Email: Download and fill out our dynamic PDF form, then email it directly to our team.
  • Electronic Medical Records (EMRs): Our forms are available on the majority of EMRs across Canada. For specific EMR instructions, please contact our dedicated EMR support at Contact – EMREACH.
